Further paperwork may be emailed to you after our staff has verified insurance coverage and details. These benefit estimations do not guarantee coverage, but are rather a courtesy service offered by Impact Sports PT to assist in understanding the cost per session for physical therapy. Final cost will be determined once your insurance company has processed claims and a patient responsibility is determined.

HELPFUL TIPS

Please follow the steps below to complete the necessary paperwork prior to your initial appointment:

  • Log-in with an email address

  • When email is confirmed, click “Review Document”

  • Enter information for all required fields

  • Complete paperwork as thoroughly as possible and submit!

  • A copy of the paperwork will be emailed to you upon submission

  • TROUBLESHOOTING: If you are having difficulty completing the forms online, particularly with Internet Explorer, please switch to Chrome
