Further paperwork may be emailed to you after our staff has verified insurance coverage and details. These benefit estimations do not guarantee coverage, but are rather a courtesy service offered by Impact Sports PT to assist in understanding the cost per session for physical therapy. Final cost will be determined once your insurance company has processed claims and a patient responsbility is determined.
HELPFUL TIPS
Please follow the steps below to complete the necessary paperwork prior to your initial appointment:
Click Fill form in top right corner
Log-in with an email address
When email is confirmed, fill a copy
Make sure box is checked for fill a copy (only you)
Check box stating "I agree to use electronic records and signature"
Click green box stating "Let's do this!"
Complete paperwork as thoroughly as possible
